CHOICE Vendor Selection Information Packets are now available from Aging and Community Services of South Central Indiana, Inc. Potential Vendors have until June 1, 2012 to complete and return the Memorandum of Agreement for CHOICE services for the elderly and persons with disabilities in Bartholomew, Brown, Decatur, Jackson and Jennings Counties. CHOICE is Indiana's Community and Home Options to Institutional Care for the Elderly and Disabled Program, which is funded by the Indiana Family and Social Services Administration. Vendors/service providers are being sought to provide services through the CHOICE Program for individual clients. Services to be purchased with CHOICE funds include adult day services, home-delivered meals, home modifications, adaptive aids and devices, homemaker, transportation and other services necessary to prevent institutionalization.
